Description

Trachea occupies the midline superior to the thoracic inlet, its C shaped hyaline cartilage rings visible anteriorly with the posterior membranous wall implied where the esophagus would lie. Inferiorly, the airway bifurcates at the carina into right and left main bronchi, with the right bronchus wider and more vertical as it courses toward the right lung hilum. Each lung presents a spongy, lobulated surface lateral to the mediastinum, with the right lung showing superior, middle, and inferior lobes separated by horizontal and oblique fissures, and the left lung showing superior and inferior lobes with an anterior cardiac notch and lingula. Medially, the hila align toward the central bronchi and pulmonary vessels, emphasizing the gateway between conducting airways and the pulmonary parenchyma. Airway caliber and branching anatomy matter at the bedside. Endotracheal tube position is judged against tracheal landmarks, and the relationship of the carina to the main bronchi explains why aspirated material more often enters the right lower lobe. Cartilage ring integrity also frames pathology, from tracheomalacia with expiratory collapse to fixed narrowing from post intubation stenosis or tumor, while the lobar and fissural layout supports surgical planning for lobectomy and segment oriented resections in bronchogenic carcinoma. Surface detail and the unobstructed thoracic context make the pleural facing contours easier to teach alongside pneumothorax or pleural effusion discussions. Orientation is immediate. Use this illustration for gross anatomy and respiratory physiology teaching, anesthesia and critical care airway modules, radiology correlation figures (CXR and CT), and surgical education addressing bronchoscopy, tracheostomy, and pulmonary resection approaches.
