Description

Concentric lamellae of compact bone surround an enlarged Haversian canal in the osteoporotic specimen, with a widened resorption space interrupting the normal continuity of the osteon and leaving thin bony septa at the periphery. On the adjacent normal model, dense cortical bone forms a uniform ring, with smaller, regular osteons and intact interstitial lamellae filling the spaces between them. The porotic voids sit centrally and extend radially, while the remaining cortex is displaced laterally and appears thinned relative to the control. Trabecular-like struts bridge some gaps but break abruptly. Bone microarchitecture matters because fracture risk tracks structure, not only bone mineral density. In osteoporosis, increased osteoclastic activity enlarges intracortical porosity and thins trabeculae, weakening resistance to bending and torsion in common injury sites such as the femoral neck, distal radius, and vertebral body. This paired presentation also supports teaching the histologic logic behind antiresorptive therapy (bisphosphonates, denosumab) versus anabolic agents (teriparatide, romosozumab), since each targets different aspects of remodeling and microstructural failure. Use this comparison in musculoskeletal anatomy and histology courses when introducing osteons, Haversian systems, and cortical versus cancellous bone, and in endocrinology or geriatrics modules addressing postmenopausal and age-related osteoporosis. It also fits well in patient education handouts and publishing layouts that need a clean visual contrast between normal cortical density and osteoporotic intracortical porosity.
