Description

Arising from the anterior surface of the mandible just inferior to the incisors, the paired mentalis muscles sit on either side of the mandibular symphysis and course inferiorly to insert into the skin of the chin. From an anterior angle in a male adult, you would expect the mentalis to be framed superiorly by the lower lip and alveolar part of the mandible, with the depressor labii inferioris positioned more laterally and the midline mental protuberance deep to the muscle belly. Bony landmarks remain prominent, including the body of the mandible and the dental arches, while the nasal aperture and orbits provide craniofacial orientation. Small muscle. Big surface effect. Functionally, mentalis elevates and protrudes the lower lip and wrinkles the skin of the chin, the classic pout, and this view makes its bilateral anatomy easy to teach as a symmetrical pair that can also act asymmetrically in facial expression. Clinically, it is a key structure in perioral surgery and cosmetic injection planning: weakness after iatrogenic injury to the marginal mandibular branch of the facial nerve can produce chin ptosis and lower lip incompetence, while overactivity contributes to the pebbled chin appearance targeted in botulinum toxin treatment. The anterior perspective also helps correlate the muscle’s bony origin with the overlying soft tissue changes seen in trauma or post-orthognathic swelling. Use this asset in head and neck anatomy teaching, facial expression modules for dental and medical curricula, and as a clear figure for texts covering facial nerve branches, perioral reconstruction, or aesthetic medicine of the chin and lower face. It also fits patient-facing education on chin dimpling, lower lip droop, and injection sites when paired with a surface anatomy diagram. Anatomical accuracy verified by SciePro's Medical Advisory Board.
