Description

Centered on a longitudinal cut through the upper gastrointestinal tract, the esophagus descends inferiorly from the pharyngeal inlet and courses posterior to the tracheal compartment (not shown) toward the stomach. At the distal esophagus, the mucosa transitions at the gastroesophageal junction into gastric cardia, with the squamocolumnar junction (Z line) implied at the interface. The stomach occupies the inferior portion of the section, with prominent gastric rugae along the mucosal surface and a thicker muscular wall laterally and inferiorly as it expands toward the body and antrum. Inflamed esophageal lining is emphasized proximally relative to the junction, contrasting with the folded gastric mucosa below. A clear boundary. Clinically, inflammation at or just proximal to the lower esophageal sphincter is the pattern most often associated with gastroesophageal reflux disease, where acid exposure produces erythema, friability, and linear erosions that radiate superiorly from the Z line. This view also supports teaching the anatomic basis for complications such as peptic stricture in the distal esophagus and Barrett esophagus, in which chronic reflux drives intestinal metaplasia at the squamocolumnar junction and changes surveillance and biopsy strategy during upper endoscopy. It also helps separate reflux-related distal disease from infectious esophagitis, which more often involves the mid to proximal esophagus in immunocompromised patients. Ideal for gastroenterology and pathology lectures covering esophagitis grading (endoscopic correlation), for patient education materials on reflux and hiatal symptoms, and for surgical texts explaining why anti-reflux procedures focus on restoring the gastroesophageal junction barrier while preserving gastric anatomy and rugae. Anatomical accuracy verified by SciePro's Medical Advisory Board.
