Description

Anteriorly, the pediatric large intestine is traced from the caecum in the right iliac fossa into the ascending colon along the right lateral abdominal wall, turning at the right colic (hepatic) flexure before crossing as the transverse colon. The bowel then descends on the left as the descending colon to the left iliac fossa, where the sigmoid colon loops medially and inferiorly toward the pelvic inlet before continuing as the rectum in the midline. Peripheral haustra and the broader framing of the colon relative to the central small bowel are suggested, with the large intestine occupying a more superficial, anterolateral course in the abdomen. Proportions reflect a male child standing in anatomical position. Pediatric colon anatomy matters because caliber, fixation, and redundancy differ from the adult pattern, and those differences shape both symptoms and imaging. A redundant sigmoid colon can predispose to volvulus, while a poorly fixed caecum and ascending colon can contribute to intermittent right lower quadrant pain in mobile caecum syndrome and complicate localization of suspected appendicitis. Clear identification of the caecum and flexures also supports teaching contrast enema reduction in intussusception, where the ileocaecal region is the target and the colonic course guides fluoroscopic progression. Landmarks count. Use this figure in pediatric anatomy and gastroenterology teaching, radiology primers that correlate abdominal pain with colonic segments, and patient education materials explaining constipation, Hirschsprung disease workup, or post-infectious colitis patterns in the large bowel. Anatomical accuracy verified by SciePro's Medical Advisory Board.
