Description

Central lobule ala (wing) occupies the superior surface of the cerebellum as a thin lateral expansion continuous with the lobulus centralis of the vermis. From the midline vermian portion it extends laterally onto the adjacent cerebellar hemisphere, lying superior to the roof of the fourth ventricle and anterior to the culmen on the superior vermis, with the precentral and postcentral fissures bracketing the central lobule along its anterior and posterior margins. Medially, the wing tapers toward the vermis; laterally, it blends into the hemispheric continuation commonly aligned with the anterior quadrangular lobule of the anterior lobe. Fissures matter here. Damage in this region is classically associated with anterior lobe and midline cerebellar syndromes, where gait and truncal ataxia can dominate, as seen with chronic alcohol-related cerebellar degeneration or compressive posterior fossa pathology. The fixed superior perspective helps you teach how the vermis anchors named lobules and how small alar extensions complicate “vermis versus hemisphere” boundaries, a common sticking point when correlating surface anatomy to MRI labels and operative notes. Surface relief and shadowing also support identification of the shallow fissures that separate the central lobule from the lingula anteriorly and the culmen posteriorly. Neuroanatomy courses, cerebellar lobulation diagrams for textbooks, and neuroradiology primers on anterior lobe landmarks are typical placements for this illustration, and it also fits neurosurgical discussions that emphasize avoiding vermian splitting while orienting to midline lobules during a suboccipital exposure.
