Description
Medial inspection of the kidney centers on the renal hilum, a concave fissure leading into the renal sinus where the renal pelvis expands and funnels inferiorly into the ureter. At the hilum, the renal vein lies most anterior, the renal artery sits posterior to it, and the renal pelvis and proximal ureter occupy the most posterior position, with segmental branches and tributaries passing into and out of the parenchyma. Superiorly and inferiorly, the hilum is bounded by the medial border of the kidney and the perihilar fat of the renal sinus, while the pedicle aligns medially toward the great vessels. Relationship matters here. This medial perspective is the workhorse view for teaching the order of the renal pedicle and for anticipating pitfalls in hilar dissection, where early branching of the renal artery or multiple renal veins can change clamp placement and increase hemorrhage risk. It also supports clinical correlation with ureteropelvic junction obstruction, where narrowing at the transition from renal pelvis to proximal ureter can be compounded by a crossing lower-pole segmental vessel at the hilum. Left-right asymmetry becomes clinically relevant as well, since the left renal vein typically courses anterior to the aorta to reach the inferior vena cava, influencing approaches for nephrectomy and renal transplantation. Use this illustration in gross anatomy and urogenital blocks to anchor spatial learning of renal artery, renal vein, and ureter at the hilum, and in surgical atlases describing open or laparoscopic hilar control, donor nephrectomy, and transplant vascular anastomoses. It also fits radiology teaching files that correlate medial renal anatomy with CT urography or angiographic planning. Anatomical accuracy verified by SciePro's Medical Advisory Board.
