Description

Anterior section through the adult female urinary bladder exposes the detrusor vesicae as a thick muscular coat encircling the vesical lumen, with the mucosa and submucosa forming rugae on the internal surface. Superiorly, the bladder dome (apex) curves toward the peritoneal reflection, while inferiorly the bladder neck narrows into the internal urethral orifice; the trigone remains a smooth triangular area between the ureteric orifices and the urethral opening. Detrusor bundles interlace and course in multiple directions, creating a functional syncytium that becomes thicker toward the base and posteroinferior wall, where the internal sphincter region in females is less discrete than in males. Spatially, the urethra continues inferiorly and slightly anteriorly from the bladder neck. Detrusor anatomy matters because coordinated contraction of this muscle, paired with urethral relaxation, is the mechanical basis of micturition, and the detrusor is the primary target in overactive bladder and neurogenic detrusor overactivity. This sectional perspective is also the clearest way to teach the distinction between the contractile detrusor and the relatively fixed trigone, a relationship that helps explain vesicoureteral reflux in congenital trigonal abnormalities and the sensory urgency seen with cystitis. Small details drive clinical reasoning. Use this artwork in gross anatomy and pelvic floor teaching sessions to orient learners to the bladder wall layers before cystoscopy, urodynamic interpretation, or urogynecologic procedures such as midurethral sling placement where bladder perforation risk hinges on understanding the bladder neck and anterior wall. It also supports medical publishing topics on detrusor hypertrophy with bladder outlet obstruction and pharmacologic targets (antimuscarinics, beta-3 agonists, intradetrusor botulinum toxin). Anatomical accuracy verified by SciePro's Medical Advisory Board.
