Description

Curving from the pylorus into the proximal small intestine, the duodenum forms a C-shaped loop whose superior, descending, horizontal (inferior), and ascending parts frame the head of the pancreas. The first (superior) part lies anterosuperior to the pancreatic head, while the second part descends along its right margin and the third part crosses horizontally at a more inferior level before turning superiorly toward the duodenojejunal flexure. Medially, the concavity of the loop relates closely to pancreatic tissue and the distal common bile duct. This spatial arrangement makes the duodenum a fixed, retroperitoneal landmark for the upper abdomen. That fixation has clinical consequences. Duodenal ulcers most often involve the duodenal bulb (proximal first part) and can erode posteriorly toward the gastroduodenal artery, a classic cause of brisk upper gastrointestinal hemorrhage. In the second part, the major duodenal papilla marks where the common bile duct and main pancreatic duct typically enter, so edema, gallstone disease, or periampullary tumors can obstruct bile flow and precipitate pancreatitis or painless jaundice. The C-loop also helps explain why pancreatic head carcinoma may narrow the descending duodenum and why surgeons reference Kocher mobilization to access the posterior duodenum and pancreatic head safely. Use this illustration for gross anatomy labs when teaching peritoneal relations and foregut derivatives, and in surgical education materials covering peptic ulcer complications, biliary obstruction, and pancreaticoduodenectomy anatomy. It also fits gastroenterology lectures and exam prep focused on the duodenal segments, papillae, and their vascular risk points. Anatomical accuracy verified by SciePro's Medical Advisory Board.
