Description
Positioned on the anterolateral male neck, the carotid triangle (trigonum caroticum) is bounded superiorly by the posterior belly of the digastric and stylohyoid, anteroinferiorly by the superior belly of the omohyoid, and posteriorly by the anterior border of the sternocleidomastoid. Within that space, the common carotid artery ascends deep to the sternocleidomastoid and bifurcates at the level of the superior border of the thyroid cartilage into the internal carotid artery (posterolateral, no cervical branches) and external carotid artery (anteromedial, giving off branches such as the superior thyroid, lingual, and facial arteries). The internal jugular vein lies lateral to the carotid arteries within the carotid sheath, while the vagus nerve courses posteriorly between artery and vein, and the hypoglossal nerve crosses the triangle superiorly as it passes toward the tongue. Key muscle planes frame the field. Clinically, this is the working anatomy for palpating the carotid pulse and for placing an incision along the anterior border of sternocleidomastoid during carotid endarterectomy. The illustration also supports teaching of carotid sinus and carotid body physiology at the bifurcation, the location most relevant to carotid sinus hypersensitivity, glomus caroticum tumors, and ultrasound evaluation of atherosclerotic plaque. You cannot plan a safe approach without respecting the relationship of cranial nerves IX to XII and the ansa cervicalis to the infrahyoid musculature. Use this artwork in head and neck anatomy courses when introducing cervical triangles, in surgical atlases covering vascular exposure of the carotid sheath, and in patient education materials explaining carotid stenosis workup by duplex Doppler.
