Description
Along the ulnar side of the palm, the opponens digiti minimi lies deep within the hypothenar eminence, positioned anterior to the fifth metacarpal and medial to the central palmar compartment. Its muscle belly originates proximally from the hamulus of the hamate and the adjacent flexor retinaculum, then runs distally and laterally to insert along the ulnar border of the shaft of metacarpal V. Superficial to it, the palmar aponeurosis and the more superficial hypothenar muscles, abductor digiti minimi and flexor digiti minimi brevis, would be expected to overlie the plane. Carpal bones at the base of the hand and the metacarpals and phalanges provide the bony framework for orientation. Opposition at the little finger often gets reduced to a teaching point, but the opponens digiti minimi matters any time you care about cupping of the hand and the transverse metacarpal arch. Loss of hypothenar bulk or weakness can follow ulnar nerve lesions, and this muscle helps you localize the deficit to the deep branch in Guyon canal when finger abduction is also weak but sensation may be spared. A small muscle, a big clue. Hand anatomy labs, peripheral nerve lectures, and operative hand surgery texts use this anterior palmar perspective to separate the layers of the hypothenar compartment and relate muscle fiber direction to the fifth carpometacarpal joint. It also fits well in patient education materials explaining ulnar neuropathy at the wrist and targeted rehabilitation for grip and key pinch patterns. Anatomical accuracy verified by SciePro's Medical Advisory Board.
