Description
Arising from the superior mesenteric artery, the middle colic artery courses superiorly into the transverse mesocolon and divides into right and left branches that track along the transverse colon near the mesenteric border. Inferior and to the right, the right colic artery runs laterally toward the ascending colon, while the ileocolic artery descends obliquely to the ileocecal region, giving ileal branches medially and colic branches that ascend along the colon. Anteriorly, the cecum and terminal ileum frame the distal ileocolic distribution, and the vascular arcades and vasa recta form a familiar laddered pattern approaching the bowel wall. Orientation is straightforward. This anterior perspective matters because colonic perfusion hinges on the continuity of the marginal artery of Drummond and the reliability of the middle colic contribution at the hepatic flexure and proximal transverse colon. Surgeons performing right hemicolectomy or extended right colectomy must identify and ligate the ileocolic, right colic (when present), and right branch of the middle colic at their mesenteric origins while preserving adequate collateral flow, a decision that directly affects anastomotic ischemia risk. The view also supports teaching of watershed vulnerability near the splenic flexure (Griffith’s point) by contrasting robust SMA territory with more variable collateralization. Ideal for gastrointestinal anatomy and general surgery curricula when introducing mesenteric vessels, lymphatics, and accompanying autonomic plexuses in relation to the colon and small bowel. It also fits colorectal operative atlases, board review material on SMA branch patterns, and patient education graphics explaining arterial supply in colon cancer resection planning. Anatomical accuracy verified by SciePro's Medical Advisory Board.
