Description

Posteriorly, the scapula dominates the field, with the scapular spine running transversely to the acromion and dividing the supraspinous fossa superiorly from the infraspinous fossa inferiorly. Lateral to the spine, the acromion overhangs the glenohumeral joint, where the humeral head sits against the glenoid cavity at the scapular neck. Portions of the clavicle are suggested anterosuperior to the acromion at the acromioclavicular region, while the upper ribs form a curved thoracic backdrop deep to the scapular body. Bony landmarks read cleanly. A posterior perspective is where scapular orientation becomes obvious, and the spine is the landmark clinicians palpate to localize the glenohumeral joint line and the posterior deltoid interval. It also frames common sources of posterior shoulder pain, including posterior labral tears and internal impingement in overhead athletes, where humeral head position relative to the glenoid matters. For surgical teaching, this angle supports discussion of posterior arthroscopic portals (posterior and posterolateral) and their relationship to the acromion and scapular spine. Use this image in gross anatomy and musculoskeletal modules to teach scapular surface anatomy and the bony architecture that underlies rotator cuff function, even when soft tissues are not rendered. It also fits orthopedic and sports medicine texts illustrating examination landmarks, portal planning, or the mechanics of glenohumeral instability.
