Description

Arising from the inner surface of the thyroid lamina, the thyroepiglottic muscle is shown coursing superiorly and posteriorly toward the lateral margin of the epiglottis, where its fibers blend with the aryepiglottic fold and adjacent intrinsic laryngeal soft tissues. Medial to it lies the laryngeal inlet, while laterally the muscle tracks along the framework of the thyroid cartilage, deep to mucosa and closely related to the paraglottic space. Superiorly, its insertion approaches the epiglottic cartilage, and inferiorly its origin anchors on the thyroid cartilage lamina. Fiber direction is clear. In a male specimen, the thyroid lamina typically sits at a more acute angle, making the relationship between cartilage contour and muscle trajectory easier to appreciate. Functionally, the thyroepiglottic muscle is often taught as an epiglottic “dilator” within the thyroarytenoid complex, counterbalancing the aryepiglottic and oblique arytenoid fibers that narrow the laryngeal inlet during swallowing. That balance matters in supraglottic airway patency and in voice, where subtle differences in supraglottic configuration contribute to strain patterns and compensatory hyperfunction. Surgeons also care about this neighborhood: the muscle’s course along the paraglottic space provides context for tumor spread in supraglottic laryngeal carcinoma and for planes encountered in endoscopic supraglottoplasty or partial laryngectomy. Use this artwork in head and neck anatomy teaching (larynx, epiglottis, intrinsic laryngeal musculature), in otolaryngology texts discussing supraglottic procedures, or in speech-language pathology materials addressing laryngeal inlet mechanics and aspiration risk.
