Description

Arising as the larger terminal branch of the tibial nerve deep to the flexor retinaculum, the medial plantar nerve courses anteriorly along the medial side of the plantar foot, accompanying the medial plantar artery between the abductor hallucis and flexor digitorum brevis. From a medial perspective, its trajectory can be followed toward the great toe compartment, where it gives digital branches to the hallux and adjacent toes and motor branches to abductor hallucis and flexor hallucis brevis. Medial to this neurovascular plane sit the calcaneus and medial longitudinal arch; more laterally lie the plantar aponeurosis and the flexor tendons as they head distally into the forefoot. Small branches cross toward the metatarsal region. Entrapment and irritation of the medial plantar nerve is a common source of medial plantar pain, often discussed under the umbrella of jogger’s foot, where repetitive friction near the navicular region and along the abductor hallucis fascia provokes burning dysesthesia in the medial sole and first three and a half digits. That topography matters when distinguishing plantar fasciitis from neuropathic pain and when planning decompression near the tarsal tunnel, because the nerve’s relationship to the flexor retinaculum, abductor hallucis, and medial calcaneal branches determines safe surgical corridors. Localization is everything. Educators can drop this view directly into lower limb anatomy teaching to clarify tibial nerve branching and the medial plantar neurovascular bundle, and authors will find it suited to chapters on tarsal tunnel syndrome, medial heel pain, and hallux sensory deficits after incision placement along the medial arch. Podiatric and sports medicine teams can also use it for patient counseling around orthotics, gait modification, and injection planning in the medial plantar compartment. Anatomical accuracy verified by SciePro's Medical Advisory Board.
